Who filmed the Apollo lunar liftoff, how was it filmed, and how was it
transmitted back to Earth?

The clip contains authentic footage of the Apollo 17 lunar module’s ascent
from the Moon that is viewable via the Smithsonian National Air and Space
Museum (bit.ly/43ZIG39).
According to the museum, the ascent was captured by a camera left behind on
the surface, controlled from Earth through an antenna on a lunar rover
vehicle that was also left behind (here).
In a transcript available through the NASA website, former Apollo flight
controller Ed Fendell describes how commands were sent to lunar rovers from
Earth, and how they were controlled (here).
Fendell’s account describes how the lunar module lift off was not captured
during the Apollo 15 mission because the camera motor “burned out,” nor
during the Apollo 16 mission because the rover was “not parked in the right
place” for the camera to capture the ascent.
