Midazolam
(Versed)
PO: Syrup, age > 6 months:
0.25 - 0.6 mg/kg as a SINGLE DOSE
of IV midazolam mixed with liquid acetaminophen or another liquid given 15 - 30 minutes before a procedure.
Note: this is IV midazolam taken as PO
May require up to 1 mg/kg.
If obese, use ideal body weight.
IV - Dilute to 0.1 mg/ml
< 6 months old: 0.025 - 0.05 mg/kg over at least 2 minutes.
6 months to 5 years: 0.05 to 0.1 mg/kg over at least 2 min.
6- 12 years: 0.025 - 0.05 mg/kg over at least 2 min.
12 - 16 years: Same as adult, but use incremental dosing
IV Only: Repeat doses in 2 minutes, if needed, in small increments of initial dose over at least 2 minutes to achieve desired endpoint. Titrate with small increments allowing 2 minutes after each dose to evaluate effect. Once sedation is achieved, additional doses should be 25% of the dose required to produce the endpoint.
Reduce dose by 30% if patient was premeditated with a narcotic or other CNS depressant.

2.b. Intermediate
Definition
Level 2.b., Intermediate level of care in the Emergency Department encompasses services that deal with patient illnesses representing a higher acuity of patient presentation. These may include presentations described in 2.a. above, which have deteriorated to some degree. Such presentations could require laboratory and radiological investigations, and medical interventions such as the provision of analgesia, conscious sedation and interventions to prevent the deterioration of a patient’s condition. Such presentations would usually result in an admission, or transfer to another institution, but fall short of intensive care admission. Trauma cases in this category are of a moderate severity and would usually require assessment by a member of the surgical and/or orthopedic team, and include complex lacerations, stable spinal fractures, compound and comminuted limb fractures.

Physician requirements:
Physician requirements in this category would ideally possess specialist qualifications in Emergency Medicine, such as board certification in Emergency Medicine or at least a post-graduate diploma qualification in Emergency Medicine.
Nursing specialty requirements:
Nursing Requirements: Nurses seeing patients in this category should have at least two years experience in managing patients presenting with moderate illness and trauma severity. Specific post-graduate qualifications in Emergency Nursing are not a pre-requisite, but would represent a distinct advantage.
